A manufacturer's or operator's formal, documented response to a confirmed failure, specifying the changes made to prevent recurrence. AECL's response to the Therac-25 findings, restoring hardware interlocks and redesigning the fault-handling interface, is an example; a documented corrective action plan is itself evidence worth checking when evaluating whether a failure's lesson was actually acted on. A plan existing is not the same as a plan working: the rigorous follow-up question is whether the change was actually implemented as described, whether it introduced any new, unintended gap of its own, and whether the incident rate for that failure mode actually dropped afterward, not just whether a document was produced and filed.
