How each discipline defines it

A concept introduced by Madeleine Clare Elish (2019) describing the pattern where a human positioned nearest an automated system's controls absorbs moral and legal blame for a failure that a fuller investigation would trace to design and organizational decisions made earlier, often because the system's design left the human too little time, information, or authority to meaningfully intervene.

It is oversight in appearance and liability laundering in fact, and drawing a boundary to create one is a governance failure.

The placement of a human in an oversight role mainly to absorb blame for a decision they had no real capacity to change; a governance anti-pattern related to the rubber-stamp, where accountability is assigned without genuine control.
