Incident log

How each discipline defines it
The same term does different work depending on who is using it. These are the definitions as each program teaches them, unedited.
A single, append-only, contemporaneously written record of every material event and decision during an active incident, with a consistent structure per entry (time, event or decision, who decided, information available, rationale). The primary artifact this topic produces; distinct from a report written after the fact from memory.
